PRACTICE INFORMATION
New Patients
We
welcome new patients at all times in our private offices
and we take walk-ins. Your primary care or consulting physician
may refer you to our office or you may call directly. In
either case, we appreciate the opportunity to service your
radiology needs.
When
you arrive at our office for the first time, please arrive
15 minutes early to complete a patient information form.
You may also choose to print this form and fill it out prior
to your arrival.
Please
bring the following with you to your first visit:

An updated insurance card
· Identification card or driver's license
Appointments
When
you arrive at our offices, you will be warmly greeted by
our receptionist and certain information will be required
from you prior to performing your exam. This information
is necessary so that we may file your claim with your insurance
company. We make every effort to maintain our schedule and
avoid any delays; however, for the convenience of all our
patients, we take walk-ins and that sometimes can affect
the schedule. We appreciate your understanding in these
circumstances.
Walk-ins
Walk-ins
are patients without a scheduled appointment and we welcome
walk-ins at all times. However, please understand that we
may not be able to always service you immediately and we
appreciate your patience.
Cancellations
In the
event you need to cancel your appointment with us, please
contact us at your earliest convenience so that we may reschedule
your exam.

Billing
And Insurance Information
We
participate with all major insurance companies in Connecticut.
Each time you visit our office, it is necessary to copy
your current insurance information. For your convenience,
we accept Mastercard/Visa, personal checks, and cash. If
your exam is covered by insurance, as a courtesy, we will
file your insurance claim for you. All unpaid balances after
insurance will be billed to you monthly. Patients are responsible
for payment of their medical care within a reasonable period
of time.
If
you do not have health insurance and cannot pay for the
service in full, we will be happy to assist you. Please
contact our billing office for assistance. If you have questions
regarding your bill, please call the business office at
